Passport Home Care


A Program Providing Options and Resources for Seniors Today

PASSPORT provides a cost-effective, in-home alternative to nursing home care for Ohio residents age 60 and older.

PASSPORT is a waiver program funded jointly by Ohio Medicaid and the federal government.

A PASSPORT assessor/ case manger will visit potential consumers for an assessment to evaluate the consumer’s ability to safely live at home.

If the consumer meets the required eligibility criteria, the assessor/ case manager and consumer will develop an individualized plan of care to assist the consumer to remain in a home setting.

 Each consumer is assigned a PASSPORT case manager who will assist the consumer and/ or their representative in organizing and monitoring their plan of care.

Eligibility Criteria These criteria are fully reviewed at the time of assessment.

An individual:

  • Must be age 60 + at the time of enrollment

  • Must meet  Medicaid financial eligibility

  • Must require Skilled or Intermediate Level of Care

  • Must  NOT  be residing in an institution at the start of services

  • Must agree to the service/ care plan

  • Service need cannot be met by other available community resources

  • Cost of services is equal to or less than the cost cap

  • Health related needs can be safely met in the home setting

  • Service plan must be approved and signed by the individual’s physician

PASSPORT is a program designed specifically for adults 60 years of age and older, promoting independence for individuals at home with long-term care needs such as:

  • Personal Care

  • Home Maker

  • Home Medical Equipment

  • Respite

  • Home Delivered  Meals

  • Dietician Services

  • Adult Day Services

  • Counseling

  • Minor Home Modifications

The program is not intended to replace what services a client currently receives, but to support those efforts.
