Full time clinical staff RN or LSW available at the Area Agency on Aging PASSPORT program. Successful applicant to provide screening, in-home assessment, and case management of home based services to qualified elders.

Requirements include either an RN or Ohio licensed LSW and at least one year of geriatrics or home health experience.

Travel required with reimbursement per mile.
