The Ohio Long-Term Care Consumer Guide provides information about nursing homes and assisted living facilities to help consumers, family members and professionals search for the appropriate facility to meet an individual’s needs. If you have determined that you need to move to a nursing or assisted living facility, you can start to compare facilities by using the Ohio Long-Term Care Consumer Guide. Please click below to begin.

We want residents' opinions about Ohio's long-term care facilities.  Nursing Home and Assisted Living Resident Satisfaction Surveys are coming soon, see the project website for more details about the

Advancing Excellence in Ohio's Nursing Homes

Be a part of Advancing Excellence in America's Nursing Home, a two-year campaign to improve nursing home care. Consumers, providers, concerned citizens and advocates are encouraged to sign on to show their support for excellence in nursing homes. Visit http://www.nhqualitycampaign.org/ to sign on to the campaign and learn more about how we all contribute to excellence in nursing care.

Residents and Families:  EXPECT EXCELLENCE!

Everyone living in long-term care should receive quality care and respect.  If you have questions or concerns about the care you or a loved one is receiving, please click here to get help from the Long-Term Care Ombudsman or call 1-800-282-1206.  To find nursing homes that have signed up for the campaign, enter Advancing Excellence in the keyword search when you search for a nursing home.

Providers:  BE EXCELLENT!

Ohio's goals for the Advancing Excellence campaign are to (1) Increase Staff Retention and (2) Improve Consistent Assignment for Staff.  Both help residents receive continuity of care from staff familiar with their needs.  Click here to learn how you can help achieve those goals.

Nursing Homes: Getting Good Care There   We are pleased to offer a free copy of Nursing Homes:  Getting Good Care There, a publication by the National Citizens’ Coalition for Nursing Home Reform.  This informative book contains valuable information about your legal rights as a nursing home resident, how to communicate with staff and what to do when you need to advocate for your loved one’s rights. Please contact us for a copy of this highly acclaimed consumer manual at consumerguide@age.state.oh.us

The Ohio Long-Term Care Consumer Guide, supported by the State of Ohio and fees paid by nursing home and assisted living facilities, contains several resources for your comparison:

Inspection Reports, from the Ohio Department of Health, provide information on nursing home compliance with state and federal law. After searching for a facility, click on the date of the last inspection to view these reports.  Updated weekly.

Facility Details, such as the special services provided, the religious or fraternal affiliations, accreditation, costs, and staffing ratios, are provided by the facilities.  After searching for a  nursing home, use the tabs along the top of the table to compare different facilities or click on an individual facility’s name for more information about it.

NEW!!  Family Satisfaction Scores, updated December 2006, reflect the opinions of family members of nursing home residents at a point in time and provide clues for you when evaluating a home for yourself or your family or friend. After searching for a nursing home, click on the link for More Survey Question Comparisons to view question by question scores and to view complete facility reports in an easy-print format.

Resident Satisfaction Survey Scores The Department of Aging will survey nursing home and assisted living facility residents during the summer of 2007.

Are you a Provider?  Please contact our office for instructions on how to register and update information about your facility on the Ohio Long-Term Care Consumer Guide.  We will send the requested information within one business day.  consumerguide@age.state.oh.us
